Seeking for $ - Need a quote on a small PHP/MySQL script

Script will be run on a intranet of a small retail/wholesale seafood business. Currently web orders come through via email, the order then needs to be sent down to the packing room to be packed and ready for the delivery drivers.

Here is what I need:

When the web order comes in, the details need to be entered into a form on the script and stored into the database when saved. The form should allow the following details:

Order Number (this needs to be free text)
Client details (should be able to select from database)
Delivery address, contact name & telephone
Area to enter what customer has ordered
Date ordered
Required delivery date & time
Order status (Not Started, In Progress, etc)

Other staff members need to see all orders and current status and be able to assign to themselves or another staff member.
When viewing all orders, status should automatically refresh at regular intervals. Orders should be printable.
There also needs to be a Admin area to setup client & staff details.

There could be other other small features that a needed, but for now if someone could please get back to me with a quote, that would be great as we're running out of time.
