Insert into form

Hi have developed a news system that can take images so what i need is a button that when clicked asks for the name of the image eg image.gif and then inserts the html code,

i know it is javascript but i have very little experiance in javascript and i have never done this before any suggestions/code?

Any help would be very much appreciated
