Linking reg form to database

Hi Im new to this, I need to bring up information from a mysql databse to a registration form, and the information to b placed in the right field of the from, any ideas anyone? Really desperate! Also once the user hits submit it shud bring up the info of the second staff memeber from the second row and so on for all 12 people!
