connect to your database:

Code:
$conn = mysql_connect('database_name', 'username', 'password');
if (!$conn) {
    die('Could not connect: ' . mysql_error());
}

create a sql query based on the fields and database name

Code:
$query = "select name,address from contact_table;";

run the query:

Code:
$run = mysql_query($query);

possibly check for failed query

Code:
if (!$row) {
echo "failed query";
}

then display the data:

Code:
while $row = mysql_fetch_array($run)) {
echo $row['name'] . " - " . $row['address'] . "\r\n";
}
