online leave application

using PHP,i want to find a way to insert the date into my database automatically when the user just fill in the form telling me the date (from & until)i mean the period that they want to take a leave.

Are you asking for ways to do it? Or do you already have some idea on what you want to do.....We will provide help, and give you ideas if need be, but it helps if you have some code you have started on. What kind of database are you insertting the info into?

In fact, i don't want the user to type the date in the form manually. But i want the user just to click the date from the pop up calendar then, then it will be inserted to the table when they click submit button.(i'm using php and mysql for my db).
if you have ready scripts will be better.

when the user fill in the form and click submit i want it to send an email in which they will have option to select an email address to send to?
