Image gallery not accepting uploads

the newer code is on my site and all the features are working except uploading an image...

here's how i customized the script:
1. renamed index.php to included.php
2. made an index.php file from a template to fit my site and included the included.php file

i've CHMOD'ed the desc.inc.php to 777 as well as the directory where my photos are stored.

i checked my server's phpinfo() and file uploads are allowed.

do you know whay it might not be allowing file uploads?

i renamed the included.php file to index.php and it worked... i'm going to have to think of another way to include the script in a template.

i tried copying the entire contents of my index.php into a template files and it's not working... this is crazy. i'll get it sooner or later.
